Code of Conduct & Medical Consent Form - posted 25.09.09
Please note that the code of conduct and medical consent forms should now have been handed out and we would appreciate it if they could be completed and returned as soon as possible. It is imperative that the conditions within the code of conduct form are fully understood by the signing parent and player. The SIHL are currently concluding their CPO policy and any amendments which we have to adopt will be relayed to all players / parents in due course.
